Pressure Ulcer Prevention and Care

Prevention is the key to warding off pressure ulcers and skin injuries.

  • Daily skin inspection

  • Moisture control

  • Proper nutrition/hydration

  • Ensure pressure ulcer assessment upon admission

  • Minimize any pressure

  • Reposition patient

  • Inspect skin around medical equipment

Proper skin care is essential for preventing and treating pressure ulcers.

  • Should be evidence-based

  • Proper nutrition/hydration given

  • Skin protective ointment for incontinence

  • Foam sacral pads for pinkish coccyx areas or other bony prominences

  • Proper antibiotics as needed.

  • Aseptic wound cleansing and care.

  • Proper documentation.

  • Wound consultation as needed.
