"4 Eyes On" includes 2 nurses doing a full head to toe skin assessment check upon admission of a patient to the floor.
According to the Health Research & Education Trust, 2016, it is crucial to have skin assessments with two nurses present. The ideal time is at nurse to nurse shift report.
